This packet will provide you with detailed information regarding the necessary steps you and your spouse (if covered under our plan) must complete in order to be compliant and receive the incentive rate for the 2022 plan year.

All requirements for the 2023 plan year must be completed and submitted to Synergy no later than 10/31/2022.

ANNUAL OPEN ENROLLMENT INFORMATION - PLAN YEAR IS JAN 1 - DEC 31, 2022

We are now approaching our Annual Open Enrollment Election period. All full time employees are required to complete their annual enrollment elections in the Kronos system. 

Annual Enrollment begins on Friday, November 12, 2021.  Enroll using Kronos (Kronos is accessible from home!)

The deadline for completing your enrollment is Friday, November 26, 2021 by 5:00 p.m. Changes will take effect on January 1, 2022 and remain in effect until December 31, 2022.

This year, the City will no longer participate in the KeyCare 20 health plan. Each employee will have a choice between the KeyCare 30 and the Lumenos HDHP. These plans and rates are explained in the rate chart further down the page.

If you participate in Flexible Spending you must re-enroll your elections for the new plan year!

Medical and Limited Medical FSA Max per year is $2750.00 or $114.58 per pay period.
Dependent Care FSA Max: Election option must not exceed a total
contribution of $5000 per Calendar year or $208.33 per pay period ($2,500 if married filing separately)

HEALTH & DENTAL RATES

The City and Schools have received the renewal information for our health and dental plans for the upcoming plan year. We will be offering 2 health insurance plan choices: KeyCare 30 and the Lumenos HDHP + HSA plans. Any employees currently enrolled in the KeyCare 20 plan need to make a choice between these two options. If a choice is not elected by the Nov. 26 deadline, KeyCare 20 participants will be automatically renewed in the KeyCare 30 plan. The rates for the health insurance and dental plans are shown below. Our health plan will continue to be administered by Anthem Blue Cross Blue Shield and the dental plan will continue to be administered by Delta Dental.

NEW LUMENOS PLAN MEMBERS - The City will continue to offer a one-time deposit to the health savings account (HSA) for any current employee that signs up for this coverage. This deposit will be $800 for the individual coverage and $1,200 for all other plan choices. This will only apply to any employee that is enrolling in the Lumenos plan for the first time during open enrollment. This deposit will be made after January 1, 2022 for all established plans.

CURRENT LUMENOS PLAN MEMBERS - The City will begin to provide an annual contribution into the HSA account for employees who have the plan as of December 31, 2021. The contribution amount will be $60 per month for subscriber only coverage ($720 per year) and $90 per month for all other lines of coverage ($1080 per year).

As always, the SALEM EMPLOYEE HEALTH & WELLNESS CLINIC located at 15 East Clay St. is open to serve your needs. USE OF THE CLINIC IS FREE FOR ALL EMPLOYEES AND DEPENDENTS AGE 2+ ON THE CITY’S HEALTH INSURANCE PLAN and can more than offset the cost of medical services you would pay at a doctor’s office, urgent care center, or emergency room. I urge you to utilize this clinic to the fullest extent possible.
